How to become an Affliate

The Reno County CDDO will gather information as to what type of services you wish to provide and answer any questions you may have regarding service provisions in Reno County. Once the affiliation agreement has been completed and signed, and all necessary paperwork is in order including obtaining a Medicaid Provider number, you will be listed on the Reno County CDDO Provider Directory and will be able to begin delivery of services to individuals with developmental disabilities in Reno County.
