A targeted case manager will provide you additional information about specific services (listed below) available in your community within Reno County. The Service Coordinator will also assist you with completion of a funding request based on determination of your needs and submit it to the Reno County CDDO for review.
The following types of services are available in Reno County through an assortment of current affiliate service providers:
Day Services provides individuals 18 years of age or older regular occurring activities that provide a sense of participation, accomplishment, personal reward and contribution.
In Home Supports include attendant care for individual's living in the family home. Care tasks may include; but not limited to, assistance with meal preparation, feeding, bathing, and daily living skills.
Home Modifications include the assessment, purchase, installation, and/or construction for adapting the individual's home (whether he or she leases, owns, or resides with his or her family) more accessible, safe, or secure for the individual.
Payroll Agent Services Payroll Agents assist individuals and/or families with varying levels of recruiting and employee management support plus the payroll and billing types of activities when they choose to direct their own support services. People typically hire, train and fire the people that provide the supports with the payroll agent managing the payroll and Medicaid billing for them. Some payroll agents also provide consultation and payroll agent services for limited license DD service providers.
Residential Services for Adults (age 18 and older) provides assistance to an individual outside the family home. Services may include assistance with personal care, household responsibilities, the preparation of food, and the social and adaptive skills necessary to enable the individual to reside in the community.
Residential Services for Children (age 5-21 years) provides placement of a child in a home other than that of his/her birth or adoptive parents. The setting must be licensed by a child-placing agency and be located near the community where the child's family lives.
Targeted Case Management Services provides service assessment, support planning, support coordination, advocacy, and transition services.
Vendors Vendors (or Equipment Vendors) are retail businesses that sell medical equipment and supplies that are often purchased by Medicaid. Allowed Medicaid waiver items are typically patient lifts, van lifts, home modification equipment to aid in person care, augmentative communication devices, etc. Approved waiver funding for these items are dependent upon availability of funds and approval by the Reno County CDDO.
Wellness Monitoring provides a nursing evaluation to determine if the recipient is properly using medical health services as recommended by a physician and if the health of the individual is sufficient to maintain him in his place of residence without more frequent skilled nursing intervention.
